OpenAI has resolved a ChatGPT incident that caused elevated file-upload and file-reading errors for several hours. The company identified the issue, applied a mitigation and monitored recovery before marking services normal at 12:39am on 3 November. Teams using documents in critical workflows should still keep retry, fallback and incident-response paths.
